- this was especially problematic when some packages were published and the the page was reloaded - it could happen that some other packages had fields filled in just because they were placed in the same position as the published packages

- js-auto-submit is only needed on checkboxes that should trigger reload of report page
- stopped autopropagation of click event on checkbox
- fixed button text not updating on deselect all

- a server error could happen when a user reported a package issue and creator emails did not exist (e.g. when harvesting) and package title had unicode characters - the error could not be reported back to user
- refactored for shorter code and to use fallback emails (exec editors if nothing else is available) instead of burdening the end user with errors

- opening the api info resulted in multiple modals opening in packages with timeseries resources (once for each year)
- browsers were issuing warnings due to duplicate ids in validation buttons

the error was due to the n index being used for timeseries resource list, but n may correspond to a longer list with additional_resources included
